Who this covers
This policy applies to Focused Health Care Services, LLC, a licensed Michigan provider of in-home care, rehabilitation, and residential services operating from Livonia and serving Southeast Michigan.
What we collect
- Information you submit. Name, phone, email, relationship to the person needing care, coverage type, and anything you write in the detail field.
- Eligibility check responses. If you use the eligibility tool, your selections are attached to your intake submission so we can respond with relevant information rather than a generic reply.
- Analytics. Aggregate data on pages viewed, time on site, general region, browser, and referring source. Not used to identify individuals.
- Cookies. Used for site function and analytics. You can disable them in your browser settings.
How we use it
- To contact you about your enquiry
- To verify coverage and build a care plan if you proceed
- To coordinate with providers, case managers, or insurers when you authorize it
- To meet Michigan legal and regulatory obligations
What we do not do
We do not sell your information. We do not share it with lead brokers, marketing networks, or unaffiliated third parties. Information goes to our own team, to service providers who operate our systems under confidentiality obligations, to parties you specifically authorize, and where legally required.
Protected health information
If we provide services to you or a family member, protected health information is handled under HIPAA and applicable Michigan regulations. A separate Notice of Privacy Practices is provided when services begin.
Your rights
You may ask what we hold, request corrections, request deletion subject to legal retention rules, and opt out of future contact. Call the number on this site to make any of those requests.
Retention
Enquiry records are kept for up to two years after last contact unless a longer period is legally required or an active care relationship exists.
Children
This site is not directed at children under 13 and we do not knowingly collect their information.

Updates will be posted here with a revised date.
